Register With The Practice?
We do not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance,
disability or medical condition. We accept requests to join the list from eligible people who have moved into the practice area. To register as a new patient, you will be asked to complete a registration form. We will also require proof of identity and entitlement to free NHS healthcare. If
you are eligible to join you will become a patient of the practice (ie not a particular GP). You may express a
preference to consult a particular GP but this cannot be guaranteed.
