How to Register

We are accepting new patients who live within our practice boundary. To register, please contact Reception on 0121 628 2345.

To minimise the spread of coronavirus (COVID-19), we are reviewing our process for registering new patients. Options we are considering including emailing or posting a registration form, and reviewing ID documents (proof of address and photographic proof of identity) if a patient needs to attend the surgery for a face-to-face appointment for the first time. Reception will be able to advise you.

You will be allocated a named GP, although you may request or be offered a telephone appointment with any available GP.

Once registered, we would usually offer you a New Patient Health Check with our health care assistant, which includes measurement of your height, weight and blood pressure, and a urine dipstick test. These health checks are being postponed during the coronavirus pandemic, but will be offered when it is once again safe for you to attend.

If you have any long-term medical conditions (e.g. asthma, diabetes or mental health conditions) or take several different medications, please consider booking a telephone appointment with a GP to set up your repeat medications and to plan your ongoing care. Please have a prescription slip or your medication boxes to hand for this appointment.
