We do not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation, appearance or medical condition. We accept requests to join the list from eligible people who live in the practice area. To register as a new patient, you will be asked to complete a registration form. We will also require proof of identity and entitlement to free NHS healthcare (NHS number) and proof of your address. If you are eligible to join you will become a patient of the practice and you will be allocated a named GP during the registration process. Having an allocated GP doesn’t guarantee that you will see that GP for every future appointment you book. We will always endeavor to comply with a request to see a specific doctor, even at short notice, however this may not always be possible.
New Patient Health Checks
If you would like a New Patient Health Check please book in with our Health Care Assistant for this. This includes measurement of your height, weight, blood pressure and a dipstick check of your urine.
If you have any long term medical conditions (such as asthma, diabetes or mental health conditions) or take several different medications, please consider booking an appointment with a GP to set up your repeat medications and plan your ongoing care. Please bring your medication boxes or a prescription slip with you to show the doctor.
Patients aged 40-74 may book in for an NHS Health Check ; this is designed to assess your risk of conditions like stroke, heart disease, diabetes, kidney disease and dementia. At the surgery we issue repeat prescriptions by sending electronically directly to your chosen chemist. We ask you to speak at reception to understand how to access our website to order repeat prescriptions over the internet.
